A thin finish coat on sound plaster. Knock on the wall: if it sounds solid, a skim is often enough.
Middle
Re-board and skim
For boards that are sagging, water-damaged or patched too many times. New boards, then a skim.
Heaviest
Hack off and re-plaster
Hollow-sounding, crumbling or blown plaster comes off to the brick, then the wall is built back up.
